Hospice of Wake CountyTransitions LifeCare Proudly Presents "An Act of Love" - Transitions LifeCare

Transitions LifeCare proudly presents An Act of Love: Talking with Your Family About Serious Illness and End of Life, a 25-minute documentary that explores the lived experiences of people in Black, African American, Hispanic, Latino and rural communities as they navigate their unique experiences with hospice and palliative care. Featuring personal stories from people in the Triangle area, the film sheds light on common misconceptions and the cultural silence that can surround end-of-life care.
Narrated by Grammy-nominated jazz artist Nnenna Freelon, the documentary amplifies a chorus of voices—families, caregivers, spiritual leaders, and healthcare professionals—offering a compassionate and multifaceted view of this critical time in life. These stories are especially vital in light of data from the National Hospice and Palliative Care Organization (NHPCO), which reveals that Black Americans on Medicare utilize hospice and palliative care 13% less frequently than their white counterparts. This disparity underscores the urgent need for culturally sensitive outreach and education.
“It is part of our mission as a nonprofit organization to provide care where it is needed,” says Adam Wolk, CEO of Transitions LifeCare. “When everyone in our community has accurate information about chronic illness care, they can make informed choices about the services they need.” An Act of Love is a powerful step toward bridging that gap—bringing awareness, empathy, and resources to historically underserved communities.
Join us for a special screening of the film on our campus in the auditorium at 250 Hospice Circle in Raleigh on June 18 from 6:00–7:30pm. To attend, make sure to RSVP to (https://bit.ly/AnActofLovePremiereRSVP). Don’t miss this moving and meaningful film—because everyone deserves guidance, resources, and compassionate care through life’s most difficult transitions.
